2. Fill up application form properly
3. Submit filled up form together with 2 pieces 1 X 1 ID picture to MSWD Office
4. Wait for the approval of ID Card
5. Releasing

1. Secure OSCA ID Application Form from the MSWD Office
2. Fill up application form properly
3. Submit filled up form together with 2 pieces 1 X 1 ID picture to MSWD Office
4. Pay the penalty fee of P100.00 to the Treasurer’s Office
5. Present Receipt to the MSWD Office
6. Wait for the approval of ID Card
7. Releasing
