Municipal Civil Registry Office

I. Functional Statement

Shall be responsible for the civil registration program in the Local Government Unit pursuant to the Civil Registry Law, the Civil Code, and other pertinent laws and regulations issued to implement them

 

II. Objectives

To maintain a systematic Archival System of the Local Civil Registry in order to preserve the records of the Civil Registry of the municipality

 

III. Programs/Projects/Activities

1. Acceptance of all registrable documents and judicial decrees affecting the civil status of the

people in the community

a. Birth

b. Marriage

c. Death

d. Fetal Death

e. Judicial Decrees

2. Filing, keeping & preserving in a secure place the books required by law

3. Transcribing all registered documents in its appropriate Civil Register

4. Issuance of Marriage License after all requirements have been complied

5. Issuance of true copies, transcriptions or certifications upon payment of prescribed fees

6. Acceptance of petitions for correction of clerical errors and change of first name in

accordance with Republic Act 9048 (Clerical Error Law)

7. Administer oath for civil registration purposes

8. Facilitate requests of authenticated copies of civil registry documents in SECPA (Security

Paper) thru BREQS (Batch Request Entry System)

9. Endorse civil registry documents to NSO-OCRG for issuance of authenticated copies in

SECPA
