Live Birth Certificate
A. ON-TIME REGISTRATION (0-30 Days from the Date of Birth)
1. Accomplished Information sheet (Form available at Civil Registry Office)
2. Marriage Contract of parents, if available
3. Item for Attendant at Birth in the Certificate of Live Birth must be duly signed by a physician,
nurse, midwife or by a traditional midwife (Hilot) who attended the birth
4. Informant should be the parent
5. If child is Illegitimate*:
a. Presence of father during registration for Admission of Paternity Purpose
b. Current Community Tax Certificate of Father
B. DELAYED REGISTRATION (Over 30 days from the Date of Birth)
1. Same as required in the ON-TIME Registration of Birth (Items1-3)
2. Any record/document wherein date and place of birth of registrant is indicated
Example: a. Baptismal Certificate
b. Voter Registration Record
c. SSS Record
d. Immunization Card (As in the case of newly born)
e. Negative Certification issued by NSO (optional)
f. Others
3. Informant should either be the parent or the registrant himself/herself
provided that he/she is 18 years old or above at the time of registration
4. Current Community Tax Certificate of Informant/Registrant
5. Marriage Contract of the Registrant, if married
6. Joint Affidavit of two disinterested persons who have known about the facts of birth of the
registrant and their current Community Tax Certificate (Joint Affidavit Form is available at
Civil Registry Office)
7. If child is Illegitimate*:
a. Informant should either be the parent or the registrant himself/herself provided that
he/she is 18 years old or above at the time of registration
b. The father is advised to be present during the registration for Admission of Paternity
purpose
b. Current Community Tax Certificate of Parents
c. Mother’s Affidavit (Affidavit if available at Civil Registry Office)
*Illegitimate - a child is said to be illegitimate if he/she was born before the marriage of parents took place.
