Certificate of Marriage
1. Either One or both of the applicants is/are bonafide residents/s of this Municipality
2. Birth Certificate of applicants
3. Accomplish Mun. Form 90 or Application for Marriage License to be filled-out by the
applicants at the Civil Registry Office
4. Applicants must undergo a marriage counseling conducted by the Pre-Marriage Counseling
Team
5. Parental Consent/Parental Advice (applicable only to applicants who are 18-24 years of age)
It must be only signed by the following order mentioned:
a. Father
b. Mother (if the father is dead; physically or mentally ill nor the father is suffering the
penalty of civil interdiction)
c. Elder brother or sister, if over 21 years of age or grandparents (if both parents are
dead; physically or mentally ill)
d. Guardian (applicable to applicants if both parents are dead, no elder brother/sister nor
grandparents, nor parents are legally prevented from exercising parental authority)
e. Person in charge (applicable to applicants with no more parents or applicants from
orphanages, children’s homes or similar institution who is a foundling, abandoned,
neglected or abused )
6. Affidavit of Legal Capacity to Marry - if the applicant is a foreigner. This can be obtained
by the applicant from his/her Embassy or Consulate based in the Philippines
7. Certificate of Death, judicial decree of absolute divorce; Judicial decree of annulment or
declaration of nullity of previous marriage or Judicial declaration that the absent spouses
presumptively dead – if the applicant has been previously married
